Geriatric Resource Nurse
A Geriatric Resource Nurse has knowledge and skill in caring for older adults. They meet with patients in their homes to assess what care is required. They review complex medical conditions and concerning changes to memory. Working collaboratively with a Geriatrician, they make suggestions for care and link the patient with resources near their home.
How do you contact the Elgin Geriatric Resource Nurse?
A referral is required from a physician or nurse practitioner. Patients are not able to self-refer to the Elgin Geriatric Resource Nurse.

Elgin Geriatric Services (BSO) is listed as Elgin Geriatric Services. Once you click on the service you want, the option to create a new referral is at the bottom of the location and services description.
