Your Stroke Care, Your Stroke Experience is a region-wide initiative designed to collect, understand, and act on the lived experiences of stroke patients across the continuum of care.  A standardized Patient-Reported Experience Measure survey is used to capture direct feedback from stroke patients to understand their experiences and identify where improvement is needed.

Our mission is to strengthen stroke care by consistently gathering patient experiences, and transforming this feedback into meaningful, region-wide improvements that advance quality, equity, and patient-centred care.
