If you have never received treatment, been a patient, or if any of your information has changed, you must report to the Central Registry Department to receive a personal hospital card.

You will be asked for info such as your legal name, preferred name, address, and next of kin.  You are also required to provide your valid Ontario Health Card.  You should also bring any other insurance information you may have such as semi-private or private room accommodation coverage.

Your visit information will be collected by the Central Registry staff for all inpatient admissions, day surgery visits, and emergency room visits.

For all other clinic visits, such as x-ray, lab, and ambulatory, you may report to the specific department at your appointment time and your registration information will be taken at that time.

Hours

Central Registry is available 24 hours a day, 7 days a week.

Location

The Central Registry Department is located on the ground floor of the hospital (near Emergency).
