Before You Leave
Your Doctor decides when you are ready to leave the hospital. Please stop at
the Nursing Station to notify staff when leaving. Remember to take a good look
around your room to ensure you have not forgotten anything. Use this checklist
to make sure you have remembered everything.
• Has the Nurse reviewed your discharge information with you?
• Do you understand what medications you will be taking at home and do you have
your prescription?
• Do you have all your personal belongings?
• Do you need information about services provided at home?
• Have you cleared up any questions you had?
• Have you paid your bills at the Patient Accounts Office?
• When you get home, if you discover that you have forgotten something at the
hospital, please call us at (519) 631-2030, and ask for the unit you stayed on.
