The Continuing Care Centre consists of 63 beds, focusing on Rehabilitation, Complex, Palliative, and Transitional Care. We provide patient-centred care for patients requiring short term or slow stream rehabilitation, pain and symptom management, complex care and patients waiting for an alternative level of care. A serene environment, with a Garden theme provides pleasant surroundings in a home-like atmosphere for patients and their loved ones.
Rehabilitation
The 10 Rehabilitation beds, located on C wing, provide support to individuals recovering from orthopedic surgery, stroke or similar conditions to transition to their highest level of physical independence. Care is planned and delivered based on goals identified in collaboration with the patient and their family.
Complex Care
The 25 Complex Care beds, located on both C & D wings, provide supportive care for chronically ill patients whose care needs exceed that which may be offered within a Long-Term care setting. Our goal is to ensure that patients are encouraged to participate in their own care. The focus of care is to support patients to regain their optimal level of physical functioning.
Palliative Care
The 8 Palliative Care beds, located on D wing, provide supportive care for patients who have a diagnosis of a progressive, ongoing life threatening illness. Patient care consists of pain and symptom management and quality of life issues that may require family support, respite care, social support and /or spiritual support.
Transitional Care
The 20 Transitional Care beds, located on B Wing, provide a supportive care environment for patients who are medically stable and are awaiting discharge either to their home or to an alternate level of care.
